When to Use

  • Collecting respondent identification
  • Registration and signup forms
  • Personalizing follow-up communications
  • Tracking individual responses

Adding a Name Question

  1. Click Add Question in the Survey Editor
  2. Select Name from the question types
  3. Enter your question text (e.g., “What is your name?”)
  4. Configure the field options
  5. Set whether the question is required

Configuration Options

Field Format

FormatFields Displayed

First, Middle & LastThree separate fields
Title, First & LastIncludes title/prefix dropdown

Field Labels

Validation

  • Required - Make the question mandatory
  • Minimum length - Set minimum characters required
  • Maximum length - Limit the number of characters

Best Practices

  1. Keep it simple - Only ask for first and last name unless you need more
  2. Consider privacy - Explain why you need their name if it’s not obvious
  3. Make it optional - Unless identification is essential, allow anonymous responses
  4. Use placeholders - Show example text like “John” and “Smith” in the fields

Data Collection

Responses are stored with clear field separation:

  • Full name responses are stored as entered
  • Split fields are stored separately for easy sorting and personalization

Use Cases

Event Registration

Collect attendee names for badges and certificates.

Customer Feedback

Identify respondents for follow-up on their feedback.

Employee Surveys

Track responses by individual (when anonymity isn’t required).

Lead Generation

Capture prospect information for sales follow-up.

Privacy Considerations

  • Name collection may require GDPR consent in some regions
  • Consider whether you truly need names or if anonymous responses would work
  • Clearly state how name data will be used

Tip: Combine Name with Email and Phone questions to create a complete contact information section.
