Contact Information

The Contact Information question gathers a respondent’s full contact details, such as name, mailing address, phone numbers, and email, in a single, organized block instead of asking for each piece separately.

When to Use

  • Registration and signup forms
  • Order or shipping details
  • Lead capture for sales follow-up
  • Any survey where you need a complete contact record in one place

Adding a Contact Information Question

  1. Click Add Question in the Survey Editor.
  2. Select Contact Information from the question types.
  3. Enter your question text (for example, “Please provide your contact details”).
  4. Choose a region format and select which fields to include.

Region Format

Under Contact Information Fields, pick a Region Format:

Choosing Fields

The configuration table lists every available field with an Enabled toggle and an editable Label:

  • First Name, Last Name
  • Address, Address 2
  • City, State, Zip Code, Country
  • Home Phone, Work Phone, Cell Phone
  • Email Address

By default, first name, last name, address, city, state, zip, country, and email are enabled, while Address 2 and the three phone fields are turned off. Toggle any field on or off to match what you need to collect. At least one field must remain enabled.

Customizing Labels

You can rename any enabled field in the Label column, for example changing “Cell Phone” to “Mobile” or “Zip Code” to a term your audience expects. The label your respondents see updates accordingly.

Best Practices

  1. Only ask for what you need to keep the form short and improve completion rates.
  2. Match the region format to your audience so state and postal fields make sense.
  3. Explain why you are collecting contact details when the reason isn’t obvious.
  • Name - Collect names on their own
  • Email - Collect and validate a single email address
  • Phone - Collect a single phone number
